It comes from an italian publication callled "Noi amiamo silvio" (We love Silvio). It is a collection of photos of "the most important successes in our Premier Berlusconi's political career". All the rest of the book is also horribly 'shopped. It's on sale in italian magazine shops

The one you pointed out is the least of the photoshopping problems in this image. Suffice to say there is no way he was there as there are no balconies in that part on Piazza Duomo nor can they be built like that. Not only the crowd, but he is badly photoshopped in himself.
Not a PSD. The camera has 360+ Degree field of view and it photographed the same group of people at both ends of the panoramic vista.

Do 360+ exist? i know 360°, but everything above should only be doable through a composite of independent images from 1 cam. But apart from that: 360°-cams don't mirror the image suddenly in its center. The "same group" is mirrored here, photographing them 2 times would allow you to align the 2 picture-parts together. 360° is also nonsense in another way: the building in the background, assumed it's not circular in reality, would never look linear if the picture was a 360°-take.

Blurry image, but you can make out that the original chunk of crowd that was cloned was to the right of 'Berlo' and so has presumably been used to pad out the crowd to the left of the photo.
It also looks like he was holding a bouquet of flowers in the original photo which had been (very) badly touched up for the magazine, along with his face and hair...
